Employee Health Insurance 2025-2026

 The following is the health insurance that any full time employee, working 30+ hours on a consistent basis for more than 60 days, can elect to receive. Please read through and if you are interested, please reach out to our main office by emailing office@qlhomecare.com with your full name and which plan you are interested in. There is a per pay deduction for this insurance and depending on which plan you chose will determine this deduction. The deduction will be either $75.77 or $95.95 per pay.

Also, your pay rate may need to be adjusted if you accept health insurance as everyone’s payrate had the health insurance amounts built into it so if you were not accepting health insurance from the beginning, you were paid a higher rate of pay. If you did, your pay rate was adjusted as needed.

If you wish to add a spouse or dependents, you will be responsible for paying for the entire premium for the additional individuals on your plan. This amount will be anywhere from $959.00 per month up to $1,638.00 per month.
